Conditions for obtaining service

  • The applicant should be licensed to practice the profession (Optometry, Contact Lenses).
  • Appointing a licensed technician to practice the profession of (Optometry, Contact Lenses) if the applicant has no time to work in the center or that the licensing applicant is not practicing the profession according to the area of work to be practiced in the center.
  • The area of the center should be no less than 12 square meters.

Procedures

  1. Referring to the Health Institutions Licensing Department and filling the licensing application of an optics center and attach the letter of the Ministry of Industry and Trade to state its purpose.
  2. Referring to the register of the Health Professions and Institutions Licensing Directorate to document the application and transfer it to the director of the Health Professions and Institutions Licensing Directorate to review and transfer the application the Health Institutions Licensing Department.
  3. Transferring the application with the required documents to the Health Institutions Licensing Department.
  4. Providing the applicant with the approval letter to register the purpose.
  5. Referring to the Health Institutions Licensing Department and providing it with the commercial register and trade name documents.
  6. Studying the application by the committee of licensing optics centers and scheduling an appointment to conduct field inspection and prepare a report about that.
  7. If the equipment of the center complies with the requirements of the regulation of optics centers licensing no.6 of 1986.
  8. Preparing an official letter signed by the Minister with approval on licensing and work commencement. 

Required Documents

  1. An official letter for the Ministry of Industry and Trade stating the purpose.
  2. Copy of practicing professions certificate for the technician or technicians in charge.
  3. Appointment letter for the technician or technicians in charge.
  4. Letter of guarantee from the technician or technicians to hold the responsibility of working in the center.
  5. Copy of title deed or lease contract.
  6. Area plan for the center.
  7. A copy of commercial register and trade name.
